Kicking dismembered heads is probably a reference to Blood. In the aformentioned game there are zombies, which can be decapitated. And their heads can be kicked. There's even a puzzle in one of the episode 1 levels, Dark Carnival, where you have to kick three dismembered zombie heads into a mouth thing to obtain a secret item. About Blood: Another build engine game, probably the best out of Duke Nukem 3D or Shadow Warrior. Was released in 1997 and made by Monolith, not 3D Realms. Didn't get as much attention as Duke Nukem 3D or Shadow Warrior. The only proper retail remaster for this game is this year's Blood: Fresh Supply, which is a recreation rather than a remaster, because it doesn't use the source code. Yes, Blood II: The Chosen exists, but most folks like to pretend it doesn't exist, like with Duke Nukem Forever.
